ABOUT OUR PRP
Afiya Cozier LLC's Psychiatric Rehabilitation Program (PRP) services for Children and Adolescents are designed to complement, support and strengthen the mental health (Therapy and Psychiatry) services the child is already receiving. A combination of one-on-one and group therapy activities are provided by our well-trained rehabilitation staffs at the Afiya Cozier LLC's, the child's home, and/or other suitable community-based places.
Both On Site (facility-based) and Off Site (community-based) services are family-centered, designed and built on the strengths and resiliency of the child and the family. Afiya Cozier LLC staff will collaborate with referral source, child and his or her family to identify specific rehabilitation goals to address identified needs.
PRP professionals regularly assess each child's progress and their parent/guardian/support team and provide monthly summary reports to the primary therapist and other mental health professionals working with the child.

Eligibility
To Be Eligible for PRP services, a child must be:

5 - 17 years old
-
Actively participating in mental health treatment
-
Referred by a Licensed Mental Health Professional
-
Diagnosed with a behavioral health diagnosis

Willing to participate in PRP services
-
A Medical Assistance recipient
